Healthcare Navigation and Guidance
  • Complete an initial comprehensive assessment of needs
  • Compile a detailed medical history and medication list to present to providers so that each has complete and concise information

  • Review current medications for any potential drug interactions or side effects and educate client on these as well as why each medication is prescribed to them and ensure their provider has all the information needed to diagnose and care for client appropriately

  • Save clients and family members time by calling to schedule appointments for client

  • Assist in arranging transportation to appointments if needed
  • Attend doctor appointments with client (virtually or in-person)
  • Assist client and family members in making a list of questions and concerns to address with healthcare providers
  • Assist in preparation for upcoming medical visits by helping to obtain medical records from different providers and completing any documentation client needs for each visit
  • Facilitate communication between healthcare providers, client and family members
  • Research physicians, second opinions, diagnoses, treatment options, medications and facilities
  • Follow up to ensure any records, labs, test results, images, etc have been sent and received by appropriate responsible parties
  • Provide research and education to help clients and their family members make the best most-informed decisions possible regarding their care plan
  • Provide resources to address client’s needs

Bedside Support

Bedside Support
  • Be present with and support client when in the ER, hospitalized or in other care facilities
  • Follow care and ensure patient safety while being treated in a healthcare facility of any type
  • Help maintain the client voice in their care, including communicating wishes to providers and family members if necessary
  • Explain diagnosis and treatment options
  • Maintain and protect the rights and dignity of the client
  • Facilitate communication between provider, client and family members
  • Communicate with case managers regarding need for assistance or home health care after discharge
  • Assist in connecting client with community resources to address needs

Transitions of Care to Another Facility or Home
  • Assist with hospital/facility admissions, stays, transfers to another facility or discharges home
  • In-person bedside monitoring or regular call-ins to facilitate communication between healthcare team and family members
  • Ensure you and/or your family members understand all discharge instructions
  • Provide resources for medical equipment and services required after discharge
  • Review any medication changes and educate client to ensure understanding and compliance with changes
  • Evaluate home environment and provide any recommendations to improve safety and ensure a safe transition to home
  • Research skilled nursing and assisted living facilities to find a good fit for client
  • Assist in making follow-up appointments, coordinating care, and arranging transportation to appointments

  • Conducting home safety assessments and providing recommendations/ resources to improve home safety
  • Assist in connecting individuals with resources for assistance with meals, transportation, activities of daily living (bathing, dressing, personal hygiene, housecleaning)
